Working From Home

After long days of anticipation and a few short weeks of scrambling with equipment and other logistics, Ruby spent her first full day working from home on Monday. Everything worked as it should for the most part.

Ruby, however, seemed to be getting dictations from the same Eastern European doctor with the thick Russian accent. After receiving her fifth dictation in a row from this same doctor, she began to grow weary and called into the office.

"How many dictations are there from this doctor anyway?" she said. She was half complaining, half whining. She was worried that some mechanism had been adjusted so that this was the only dictator she would be receiving while working from home. "Just a few more." was the reply.

Hambut, always the joker, sent her the following E-mail.

HISTORY:
I was asked by Dr. Hambone to visit this patient for unclear reasons. She was reportedly having auditory and visual hallucinations and exhibiting agitated and violent behavior. . From the ER record I learned that the patient grabbed her grandmother this morning, which strongly suggests a grandma seizure. Patient states she sees women with red hats on occasion, but not too often. She also complains of voices in her headphones, but that the voices do not always make sense to her. This is frustrating to her because the voices tell her to write down what they say. Her friends report that most recently she has become more isolated, preferring to stay in her home in Longview.

Miss Hardbottle is a pleasant, well-developed, relatively well-adjusted Caucasian female with a cheerful mood and affect. . She does not smoke, rarely drinks, and does not use illegal drugs, though I detect a hint of chocolate on her breath.

HEENT: Strict eyes and nose. Cute face, nice smile, eyes dilate - and it is better to dilate than early.

Patient declined the remainder of the physical examination, and Dr. Proudfoot does not want any legal problems. :)

ASSESSMENT & PLAN:
At this time the patient is not dangerous to herself or others unless she drives in fog. As there is no fog today, she should be discharged. An ITA hold is not necessary, but a hug might help.

Patient may follow up whenever she likes.
